Welcome to Lively! First things first, let's get the paperwork out of the way, though. Before you can start work, you'll need to go through the steps below.
Before making a formal offer, please submit the following checks, which may be required by law or by our partners who refer clients to Lively:

Working with Children Check
If you are citizen of a country other than Australia, a statutory declaration stating that you do not have a criminal history in another country. This is still required if you also hold Australian citizenship.
If you already have any of these documents, you can easily provide them to Lively via the 'Add Check' form. But if not, don't stress! Please follow the instructions below.
If you do not have a Police Check, ask the Lively Hiring team (hiring@lively.org.au) to request one on your behalf. This will then be billed to Lively.
You will receive an email from Employment Screening Group, and approved provider of criminal history checks, who conducts screenings on Lively's behalf.
You will need to provide a number of forms of ID.
When asked, list the following:
Vulnerable group = Aged
Contact = Unsupervised contact
When the results come in, please upload them via the button above.
If you do not have a Working with Children Check, you will need to request one via Service Victoria here.

The cost of this check will be around $130–140, which you can submit as an expense to Lively here, and we will reimburse this amount to you on your first payroll.

A statutory declaration is a written statement that you sign in the presence of an authorised person.
Please print off the template to the right.
Sign it in front of an authorised person. You can find a list of professions that can witness your signature here. The easiest option may be to visit an Australia Post, where there is typically someone who can witness statutory declarations.
Take a photo of your statement and upload the image via the form above.
We will also conduct the following checks on your behalf:
Banning order check: to make sure you have not been formally excluded from working with older people)
VEVO check (if you are on a visa): to ensure you have no work restrictions that might affect your work with us.
Note: we may commence your training before the Checks process is finalised, but you will not be able to start work until all checks are complete.
The offer will be subject to clearance of the above checks. Following clearance, we will send you a formal employment contract. You will receive this document via email from our e-signature app.

Before your first training session, please access and download the Lively app by visiting app.lively.org.au on your phone and following the prompts. Make sure you sign up using the email you use for your Lively correspondence, as we will be using this for your account login.
When you sign up to the app, you will be prompted to complete our onboarding form to provide us with your essential information, including your contact details, relevant health conditions, pay and superannuation preferences. You'll need to have the following information handy:
Photo of yourself (upload from your phone)
COVID-19 vaccination certificate (upload from your phone)
Superannuation fund details (or you can choose to use Lively's nominated super fund)
Bank details
Tax file number (TFN)
